ABSTRACT Background: Rapid correction of hyponatremia, especially when severe and chronic, can result in osmotic demyelination. The latest guideline for diagnosis and treatment of hyponatremia (2014) recommends a correction limit of 10 mEq/L/day. Our aim was to summarize published cases of osmotic demyelination to assess the adequacy of this recommendation. Method: Systematic review of case reports of osmotic demyelination. We included cases confirmed by imaging or pathology exam, in people over 18 years of age, published between 1997 and 2019, in English or Portuguese. Results: We evaluated 96 cases of osmotic demyelination, 58.3% female, with a mean age of 48.2 ± 12.9 years. Median admission serum sodium was 105 mEq/L and > 90% of patients had severe hyponatremia (<120 mEq/L). Reports of gastrointestinal tract disorders (38.5%), alcoholism (31.3%) and use of diuretics (27%) were common. Correction of hyponatremia was performed mainly with isotonic (46.9%) or hypertonic (33.7%) saline solution. Correction of associated hypokalemia occurred in 18.8%. In 66.6% of cases there was correction of natremia above 10 mEq/L on the first day of hospitalization; the rate was not reported in 22.9% and in only 10.4% was it less than 10 mEq/L/day. Conclusion: The development of osmotic demyelination was predominant in women under 50 years of age, with severe hyponatremia and rapid correction. In 10.4% of cases, there was demyelination even with correction <10 mEq/L/day. These data reinforce the need for conservative targets for high-risk patients, such as 4-6 mEq/L/day, not exceeding the limit of 8 mEq/L/day.


RESUMO Antecedentes: A correção rápida da hiponatremia, principalmente quando grave e crônica, pode resultar em desmielinização osmótica. A última diretriz para diagnóstico e tratamento da hiponatremia (2014) recomenda um limite de correção de 10 mEq/L/dia. Nosso objetivo foi sumarizar os casos publicados de desmielinização osmótica para avaliar a adequação dessa recomendação. Método: Revisão sistemática de relatos de caso de desmielinização osmótica. Incluímos casos confirmados por imagem ou anatomia patológica, em maiores de 18 anos, publicados entre 1997 e 2019, nas línguas inglesa ou portuguesa. Resultados: Avaliamos 96 casos de desmielinização osmótica, sendo 58,3% do sexo feminino e com média de idade de 48,2 ± 12,9 anos. A mediana de sódio sérico admissional foi 105 mEq/L e > 90% dos pacientes apresentavam hiponatremia grave (<120 mEq/L). Foram comuns os relatos de distúrbios do trato gastrointestinal (38,5%), etilismo (31,3%) e uso de diuréticos (27%). A correção da hiponatremia foi feita principalmente com solução salina isotônica (46,9%) ou hipertônica (33,7%). Correção de hipocalemia associada ocorreu em 18,8%. Em 66,6% dos casos houve correção da natremia acima de 10 mEq/L no primeiro dia de internamento; a velocidade não foi relatada em 22,9% e em apenas 10,4% foi menor que 10 mEq/L/dia. Conclusão: O desenvolvimento da desmielinização osmótica foi predominante em mulheres, abaixo de 50 anos, com hiponatremia grave e correção rápida. Em 10,4% dos casos, houve desmielinização mesmo com correção <10 mEq/L/dia. Esses dados reforçam a necessidade de alvos conservadores para pacientes de alto risco, como 4-6 mEq/L/dia, não ultrapassando o limite de 8 mEq/L/dia.

ABSTRACT

El síndrome de antiduresis inadecuada (SADI) se produce por una alteración en el eje hipotálamo-neurohipófisis, por una falla en la respuesta regulatoria osmótica o por factores no osmóticos, con complicaciones en relación directa al grado de hiponatremia e impacto en la calidad de vida y mortalidad del paciente. El tratamiento consiste en la normalización de la natremia, y la búsqueda de la etiología. Presentamos el caso de un paciente adulto con el diagnóstico de SADI idiopático y trastorno neurocognitivo asociado, con respuesta favorable al tratamiento con urea.


Syndrome of Inappropriate Antidiuresis (SIAD), is produced by an alteration in the hypothalamus-neurohypophysis axis due to a failure in the osmotic regulatory response or non-osmotic factors, with complications directly related to the degree of hyponatremia and impact on quality of life and patient mortality. Management consists of normalization of natremia, and the search for the underlying etiology. We present the case of an adult patient diagnosed with idiopathic SIAD and associated neurocognitive disorder, with a favorable response to treatment with urea.

ABSTRACT

El síndrome de absorción intravascular en histeroscopia se origina por la rápida absorción vascular de soluciones isotónicas e hipotónicas utilizadas en irrigación intrauterina, ocasionando hipervolemia y dilución de electrolitos, especialmente hiponatremia. Cuando este síndrome es debido a intoxicación por glicina al 1,5% causa acidosis severa y neurotoxicidad. La incidencia de este síndrome es baja pero puede aumentar por factores como: falta de control de altura de bolsas de irrigación, ausencia de equilibrio de fluidos de soluciones de irrigación, tejidos altamente vascularizados como miomas uterinos y uso de sistema de electrocirugía monopolar. Se reporta el caso de una paciente con miomas uterinos, programada para resección mediante histeroscopia que cursa con síndrome de absorción intravascular por glicina, el temprano diagnóstico y rápido tratamiento intraoperatorio y postoperatorio permitió una evolución favorable. El manejo se basó en el uso de diuréticos, restricción de fluidos y soluciones hipertónicas de sodio.


Intravascular absorption syndrome in hysteroscopy is caused by rapid vascular absorption of isotonic and hypotonic solutions used in intrauterine irrigation, causing hypervolemia and electrolyte dilution, especially hyponatremia. When this syndrome is due to 1.5% glycine toxicity, it causes severe acidosis and neurotoxicity. The incidence of this syndrome is low but may increase due to factors such as: lack of control of the height of irrigation bags, lack of fluid balance in irrigation solutions, highly vascularized tissues such as uterine myomas and use of a monopolar electrosurgery system. The case of a patient with uterine myomas, scheduled for resection by hysteroscopy, who presents with intravascular glycine absorption syndrome, is reported. Early diagnosis and rapid intraoperative and postoperative treatment allowed a favorable evolution. Management was based on the use of diuretics, fluid restriction, and hypertonic sodium solutions.

ABSTRACT

Se presenta el caso de un paciente varón de 55 años que ingresó por alteración del estado de conciencia por una hiponatremia severa secundaria a una meningitis tuberculosa. No hubo mejoría de la hiponatremia al tratamiento con solución salina hipertónica, por lo cual se planteó el diagnóstico de síndrome de secreción inapropiada de hormona antidiurética (SIADH) y se evidenció mejoría con la restricción hídrica. El interés del presente caso es reportar una complicación frecuente pero olvidada de la meningitis tuberculosa.


We present the case of a 55-year-old male patient who was admitted due to an altered state of consciousness due to severe hyponatremia secondary to tuberculous meningitis. There was no improvement in hyponatremia after treatment with hypertonic saline solution, therefore the diagnosis of syndrome of inappropriate secretion of antidiuretic hormone (SIADH) was proposed, and improvement was evidenced with fluid restriction. The interest of this case is to report a common but forgotten complication of tuberculous meningitis.

ABSTRACT

La encefalitis límbica (EL) autoinmune es una afección neurológica infrecuente de curso subagudo con manifestaciones neuropsicológicas. Actualmente el tratamiento inmunoterápico agudo o de mantenimiento es dirigido según el anticuerpo neural acompañante y la presencia o ausencia de cáncer. Presentamos el caso de una mujer de 52 años con hipotiroidismo autoinmune, síndrome de secreción inadecuada de hormona antidiurética (SIADH) e hiponatremia (hipoNa) persistente, con evolución progresiva de perdida de la memoria y crisis distónicas faciobraquiales (DFBC) a quien se le realiza un diagnóstico oportuno de encefalitis límbica. Recibió tratamiento intravenoso combinado en base a corticoides e inmunoglobulina con buena respuesta y morbilidad mínima neuropsicológica. El reconocimiento de esta patología permite un diagnóstico y tratamiento temprano, imprescindible para mejorar el pronóstico de estos pacientes.


Autoimmune limbic encephalitis is a rather unusual neurological condition with subacute progression and neuropsychological symptoms. Currently, acute or maintenance treatment with immunotherapy is targeted depending on the accompanying neural specific antibody and the presence or absence of cancer. The study presents the case of a 52-year-old woman suffering from autoimmune hypothyroidism, syndrome of inappropriate secretion of anti-diuretic hormone (SIADH) and persistent hyponatremia, with progressive evolution which involved memory loss and faciobrachial dystonic seizures (FBDS). She was timely diagnosed with limbic encephalitis and was treated with intravenous combined corticosteroids and immunoglobulin therapy. Response was good, with minimum neuropsychological. Recognizing this condition allows for early diagnosis and treatment, what is key to improve the prognosis of these patients.


A encefalite límbica (LE) autoimune é uma condição neurológica rara de curso subagudo com manifestações neuropsiquiátrica. Atualmente, o tratamento com imunoterapia aguda ou de manutenção é orientado de acordo com o anticorpo neural e a presença ou ausência de câncer. Apresentamos o caso de uma mulher de 52 anos com hipotireoidismo autoimune, síndrome de secreção inapropriada de hormônio antidiurético e hiponatremia persistente, com evolução progressiva da perda de memória e crises distônicas faciobraquiais que foi diagnosticada oportunamente como encefalite límbica. Recebeu tratamento endovenoso combinado à base de corticoide e imunoglobulina com boa resposta e morbidade neuropsiquiátrica mínima. O reconhecimento desta patologia permite um diagnóstico e tratamento precoces, essenciais para melhorar o prognóstico desses pacientes.

Introducción: El síndrome de desmielinización osmótica es una entidad clínica la cual supone un reto diagnóstico por su curso en muchos casos silentes y por su cuadro clínico que se presenta típicamente de manera tardía. Se describen dos tipos de desmielinización osmótica que pueden presentarse de manera aislada o simultánea: la mielinosis central pontina y la mielinosis extrapontina. Caso clínico: femenina de 21 años sin antecedentes personales con episodio convulsivo tónico clónico por primera vez en su vida. Se le encuentra a su ingreso un nivel de sodio sérico disminuido, razón por la cual se inicia la corrección intravenosa del mismo. Cursó con cefaleas persistentes y vómitos a las 48 horas posterior a la corrección del sodio, y se realizó una resonancia magnética cerebral que evidenció datos de mielinosis extrapontina. Conclusión: La corrección rápida de los niveles de sodio se asocia a la posibilidad de desarrollar mielinosis pontina. La precaución en la corrección de la hiponatremia es la piedra angular para prevenir el desarrollo de esta condición. (provisto por Infomedic International)


Introduction: Osmotic demyelination syndrome is a clinical entity which poses a diagnostic challenge because of its often-silent course and its clinical picture which typically presents late. We describe two types of osmotic demyelination that can occur in isolation or simultaneously: central pontine myelinosis and extrapontine myelinosis. Case report: 21-year-old female with no personal history with tonic-clonic seizure episode for the first time in her life. On admission she was found to have a decreased serum sodium level, for which reason intravenous sodium correction was started. He presented with persistent headaches and vomiting 48 hours after sodium correction, and a brain MRI was performed, which showed evidence of extrapontine myelinosis. Conclusion: Rapid correction of sodium levels is associated with the possibility of developing pontine myelinosis. Caution in correcting hyponatremia is the cornerstone in preventing the development of this condition. (provided by Infomedic International)

Contexto la hiponatremia posoperatoria es una complicación frecuente que se encuentra hasta en el 4 % de los pacientes sometidos a cirugía y que además es un factor independiente de mortalidad intrahospitalaria. Hasta ahora, los estudios se enfocan en las primeras horas posteriores a la cirugía, existiendo poca literatura que abarque hasta el periodo posoperatorio tardío. Objetivo evaluar la frecuencia y las características de hiponatremia en la primera semana posoperatoria. Metodología se incluyeron pacientes sometidos a cirugía mayor durante un periodo de 18 meses. Se determinó el nivel de sodio a las 24 horas, al tercer, quinto y séptimo día del procedimiento. Se clasificó de acuerdo con el tipo de cirugía realizada y se documentó la presencia de datos clínicos y desenlaces de los pacientes. Resultados 280 pacientes fueron estudiados, de los cuales 20 (7,1 %) desarrollaron hiponatremia durante el primer día posoperatorio, 34 (12,1 %) en el tercero, 30 (10,7 %) en el quinto día y 31 (11 %) en el séptimo día. En el primer día posoperatorio, los pacientes sometidos a cirugías urológicas y gastrointestinales desarrollaron hiponatremia con mayor frecuencia. Ninguno de los pacientes desarrolló complicaciones graves y no hubo relación entre la edad, el género o el tipo de soluciones y el desarrollo de hiponatremia. Conclusiones la hiponatremia es una complicación frecuente de varios tipos de procedimientos quirúrgicos que puede desarrollarse en los primeros siete días posoperatorios.


Background Postoperative hyponatremia is a frequent complication that is found in more than 4% of patients undergoing surgery. It is also an independent factor of in-hospital mortality. Until now, previous studies have focused on the first hours after surgery, so there is little information regarding hyponatremia in the late postoperative period. Purpose evaluate the frequency and characteristics of hyponatremia in the first postsurgical week. Methodology Patients undergoing major surgery over a period of 18 months were included. Sodium level was determined at 24 hours, third, fifth and seventh day of the procedure. Hyponatremia was classified according to surgery type and presence of clinical symptoms and outcomes were documented. Results 280 patients were studied. 20 (7.1%) developed hyponatremia on the first postoperative day, 34 (12.1%) on the third, 30 (10.7%) on the fifth day, and 31 (11%) on the seventh day. On the first postoperative day, patients undergoing urological and gastrointestinal surgeries developed hyponatremia more frequently. None of the patients developed serious complications. There was no relationship between age, gender or type of solutions and the development of hyponatremia. Conclusions hyponatremia is a frequent complication of surgical procedures that can develop in the first seven postoperative days.

Resumo Fundamento A coexistência de hiponatremia e fibrilação atrial (FA) aumenta a morbidade e mortalidade em pacientes com insuficiência cardíaca (IC). No entanto, não está estabelecido se a hiponatremia está relacionada à FA ou não. Objetivo O objetivo do nosso estudo foi buscar a possível associação de hiponatremia com FA em pacientes que apresentam IC com fração de ejeção reduzida (ICFrE). Métodos Este estudo observacional, transversal e unicêntrico incluiu 280 pacientes ambulatoriais consecutivos com diagnóstico de ICFr com 40% ou menos. Com base nas concentrações de sódio ≤135 mEq/L ou superior, os pacientes foram classificados em hiponatremia (n=66) e normonatremia (n=214). Um valor de p<0,05 foi considerado significativo. Resultados A média de idade foi de 67,6±10,5 anos, 202 (72,2%) eram do sexo masculino, o nível médio de sódio no sangue foi de 138±3,6 mEq/L e a fração de ejeção média foi de 30±4%. Ao todo, 195 (69,6%) pacientes foram diagnosticados com doença arterial coronariana. A FA foi detectada em 124 (44.3%) pacientes. A taxa de FA foi maior em pacientes com hiponatremia em comparação com aqueles com normonatremia (n=39 [59,1%] vs. n=85 [39,7%), p=0,020). Na análise de regressão logística, a hiponatremia não foi relacionada à FA (OR=1.022, IC 95%=0,785-1.330, p=0,871). Idade aumentada (OR=1.046, IC 95%=1.016-1.177, p=0,003), presença de DAC (OR=2.058, IC 95%=1,122-3.777, p=0,020), frequência cardíaca em repouso (OR=1.041, IC 95%=1.023-1.060, p<0,001) e diâmetro do átrio esquerdo (OR=1.049, IC 95%=1.011-1.616, p=0,002) foram considerados preditores de FA. Conclusão A FA foi uma taxa mais elevada em pacientes ambulatoriais com ICFr e hiponatremia. No entanto, não há associação entre os níveis de sódio e FA em pacientes com ICFrEF.


Abstract Background The coexistence of hyponatremia and atrial fibrillation (AF) increases morbidity and mortality in patients with heart failure (HF). However, it is not established whether hyponatremia is related to AF or not. Objective Our study aims to seek a potential association of hyponatremia with AF in patients with reduced ejection fraction heart failure (HFrEF). Methods This observational cross-sectional single-center study included 280 consecutive outpatients diagnosed with HFrEF with 40% or less. Based on sodium concentrations ≤135 mEq/L or higher, the patients were classified into hyponatremia (n=66) and normonatremia (n=214). A p-value <0.05 was considered significant. Results Mean age was 67.6±10.5 years, 202 of them (72.2%) were male, mean blood sodium level was 138±3.6 mEq/L, and mean ejection fraction was 30±4%. Of those, 195 (69.6%) patients were diagnosed with coronary artery disease. AF was detected in 124 (44.3%) patients. AF rate was higher in patients with hyponatremia compared to those with normonatremia (n=39 [59.1%] vs. n=85 [39.7%), p= 0.020). In the logistic regression analysis, hyponatremia was not related to AF (OR=1.022, 95% CI=0.785-1.330, p=0.871). Advanced age (OR=1.046, 95% CI=1.016-1.177, p=0.003), presence of CAD (OR=2.058, 95% CI=1.122-3.777, p=0.020), resting heart rate (OR=1.041, 95% CI=1.023-1.060, p<0.001), and left atrium diameter (OR=1.049, 95% CI=1.011-1.616, p=0.002) were found to be predictors of AF. Conclusion AF was higher in outpatients with HFrEF and hyponatremia. However, there is no association between sodium levels and AF in patients with HFrEF.

ABSTRACT

Mujer de 73 años con antecedente de epilepsia, deterioro cognitivo, rigidez y movimientos involuntarios. Ingresó por emergencia con cuadro de desorientación y dificultad del habla. La TEM cerebral mostró calcificaciones en los ganglios basales y hemisferios cerebelosos sugerentes de síndrome de Fahr. Se demostró un hipoparatiroidismo primario más enfermedad tiroidea autoinmune con una hiponatremia hipotónica euvolémica y sin alteración de otros ejes hipofisiarios. Se hizo el diagnóstico de síndrome poliglandular autoinmune tipo 4; además, el dosaje de ADH confirmó el diagnóstico de síndrome de secreción inadecuada de hormona antidiurética. Se administró calcio endovenoso, suplementos de vitamina D y calcitriol, con mejoría clínica neurológica importante.

Resumen La crisis adrenal es la forma más extrema de presentación de la insuficiencia adrenal y representa una urgencia endocrinológica que llega a poner en riesgo la vida. Esta situación puede ser des encadenada por diferentes causas, entre las cuales se incluye el uso de fármacos inductores del CYP3A4, que aceleran la depuración de la hidrocortisona. Describimos el caso de una mujer de 85 años, con antecedentes de insuficiencia adrenal secundaria y enfermedad renal crónica, que presentó síntomas compatibles con crisis adrenal (astenia, adinamia, hiponatremia grave con síntomas neurológicos e hipotensión arterial) luego de nueve días del inicio de tratamiento con modafinilo. El cuadro clínico mejoró rápidamente con la suspensión del modafinilo y la administración de hidrocortisona endovenosa. Luego de descartar las posibles causas desencadenantes (infecciosas, isquémicas, tromboembolismo pulmonar y omisión en la toma de hidrocortisona), se interpretó que el modafinilo precipitó los síntomas de insuficiencia adrenal al aumentar la depuración del corticoide. El modafinilo tiene la capacidad de inducir la actividad del CYP3A4 y, en consecuencia, disminuir la biodisponibilidad de la hidrocortisona. Recalcamos la necesidad de ajustar la dosis de reemplazo de corticoides en sujetos que reciben fármacos inductores del metabolismo.


Abstract Adrenal crisis is the most extreme presentation form of adrenal insufficiency and represents a life-threatening endocrinological emergency. This situation can be triggered by different causes including the use of CYP3A4-inducing drugs, which accelerate hydrocortisone clearance. We describe the case of an 85-year-old woman with secondary adrenal insufficiency and chronic renal disease, who presented symptoms compatible with adrenal crisis (asthenia, adynamia, severe hyponatremia associated with neurological symptoms and hypotension) nine days after the start of modafinil treat ment. The clinical picture improved rapidly with the suspension of modafinil and the administration of intravenous hydrocortisone. After ruling out the possible triggering causes (infectious, ischemic, pulmonary thromboembo lism and failure to take hydrocortisone), it was interpreted that modafinil precipitated the symptoms of adrenal insufficiency by increasing the steroid clearance. Modafinil has the ability to induce the activity of CYP3A4 and consequently decrease the bioavailability of hydrocortisone. We emphasize the need to adjust steroid dose re placement in subjects receiving metabolism-inducing drugs.

Introducción: La hiponatremia es la alteración electrolítica más frecuente en el paciente geriátrico. Existen evidencias que la asocian a un peor pronóstico en pacientes con insuficiencia cardiaca. Objetivo: Caracterizar los pacientes geriátricos ingresados con hiponatremia al ingreso e insuficiencia cardíaca. Método: Se realizó un estudio descriptivo, longitudinal y prospectivo durante el año 2018 en el Hospital Universitario Clínico Quirúrgico Calixto García que incluyó 260 pacientes con insuficiencia cardíaca e hiponatremia al ingreso. Para el análisis estadístico de los datos reutilizaron la prueba de chi cuadrada y el análisis multivariado de ANOVA para la asociación entre variables. Resultados: La edad media fue 72,6 ± 8,2, predominaron las mujeres (55,0 por ciento). Prevaleció la puntuación de Charlson 3-4 (33,8 por ciento), la fracción de eyección conservada, 70,8 por ciento; clase funcional III, 33,8 por ciento; estadía menor a 6 días, 43,1 por ciento y fallecieron 51,9 por ciento de la muestra estudiada. Se asociaron significativamente con la mortalidad al egreso, el índice de comorbilidad y la clase funcional III-IV, p< 0,05. Conclusiones: Existe una elevada mortalidad en pacientes geriátricos hospitalizados por insuficiencia cardiaca e hiponatremia al ingreso asociada a la presencia de comorbilidad y a la clasificación de la insuficiencia cardiaca(AU)


Introduction: Hyponatremia is the most frequent electrolyte alteration in geriatric patients. There is evidence that associates it with a worse prognosis in patients with heart failure. Objective: To describe geriatric patients admitted with hyponatremia on admission and heart failure. Method: A descriptive, longitudinal and prospective study was carried out in 2018 at Calixto García Surgical Clinical University Hospital, including 260 patients with heart failure and hyponatremia on admission. For the statistical analysis of the data, they reused the chi-square test and the multivariate analysis of ANOVA for the association between variables. Results: The mean age was 72.6 ± 8.2, women predominated (55.0 percent). Prevalence was observed in the Charlson score 3-4 (33.8 percent), the ejection fraction preserved (70.8 percent); functional class III was 33.8 percent; 43.1 percent stayed less than 6 days and 51.9 percent of the sample studied died. They were significantly associated with mortality at discharge, the comorbidity index and functional class III-IV, p <0.05. Conclusions: There is a high mortality in geriatric patients hospitalized for heart failure and hyponatremia on admission associated with the presence of comorbidity and the classification of heart failure(AU)

Introducción: Para disminuir la aparición de hiponatremias en los últimos años se aumentaron las concentraciones de sodio en las soluciones de mantenimiento, llegando a recomendarse las isotónicas, con mejoras de laboratorio pero con dudoso impacto clínico. En el Hospital Garrahan se utiliza una solución estándar hipotónica con cloruro de sodio 0,45%. Antes de reemplazar la solución según recomendaciones internacionales se decidió establecer la prevalencia de hiponatremia en pacientes internados, y su asociación con la solución estándar de hidratación. Población y métodos: Pacientes de 1 mes a 18 años, internados en el Hospital Garrahan. Estudio prospectivo y observacional. Se registró si el paciente recibía hidratación parenteral y la concentración de sodio. Se consideró hiponatremia significativa la presencia de sodio sérico menor a 130 mEq/L. y/o la presencia de síntomas compatibles con hiponatremia. Resultados: En 3003 internaciones la prevalencia global de hiponatremias diagnosticadas fue 4.4%, y asciende a 6.3% si se consideran solo los pacientes que fueron estudiados con ionograma (se le extrajo ionograma al 70,6% de los pacientes internados). La prevalencia de hiponatremias significativas fue de 1.5% (n=44) de los internados, y las hiponatremias significativas en internados que recibían la solución hipotónica estándar de mantenimiento fue de 0.3% (n=9). Conclusiones: En una población donde se utiliza una solución estándar con cloruro de sodio 0,45% -pero se modifica individualmente para las necesidades de cada paciente- la prevalencia de hiponatremias totales y significativas fue similar e incluso inferior a la publicada en otras series. Introduction: To reduce the appearance of hyponatremia, in recent years, sodium concentrations were increased in maintenance solutions, and isotonic solutions were recommended, leading to improvements in laboratory studies, but with a doubtful clinical impact. A standard hypotonic solution with 0.45% sodium chloride is used at Garrahan Hospital. Before replacing the solution according to international recommendations, it was decided to determine the prevalence of hyponatremia in inpatients and its association with the standard hydration solution. Population and methods: Patients from 1 month to 18 years old, hospitalized at Garrahan Hospital. Prospective and observational study. Parenteral hydration of the patient and the sodium concentration were recorded. Significant hyponatremia was defined as serum sodium less than 130 mEq/L, and/or the presence of symptoms of hyponatremia. Results: In 3003 hospitalizations, the overall prevalence of diagnosed hyponatremia was 4.4%, increasing to 6.3% if only patients in whom a ionogram was performed were included (a ionogram was performed in 70.6% of the inpatients). Of all inpatients, 1.5% (n=44) had significant hyponatremia, and 0.3% (n=9) of the patients receiving the standard maintenance hypotonic solution had significant hyponatremia. Conclusions: In a population in whom a standard solution with 0.45% sodium chloride is used - but which is individually modified according to the needs of each patient - the prevalence of total and significant hyponatremia was similar and even lower than that reported in other series (AU)

ABSTRACT

O objetivo deste estudo foi evidenciar e discutir as principais alterações hidroeletrolíticas em pessoas com cirrose. Trata-se de uma revisão integrativa, de natureza qualitativa. Os artigos foram selecionados por meio da plataforma Medical Literature Analysis and Retrievel System Online. Os principais achados identificados a partir dos artigos selecionados foram a ocorrência de hiponatremia, o mau prognóstico diante da presença de distúrbios hidroeletrolíticos em relação à sobrevida em pessoas com cirrose e a importância da albumina. Indivíduos com cirrose são suscetíveis ao desenvolvimento de distúrbios hidroeletrolíticos devido às mudanças fisiopatológicas da doença e às condições clínicas apresentadas. A hiponatremia e a hipocalemia são os mais recorrentes, destacando, porém, a necessidade de atenção aos demais distúrbios. (AU)


The objective of this study was to show and discuss the main hydroelectrolytic alterations in cirrhotic patients. This is an integrative review, a qualitative study, in which articles were selected at the Medical literature Analysis and Retrieval System Online. The main findings identified in the articles selected were the occurrence of hyponatremia, the poor prognostic, due to the presence of hydroelectrolytic disorders, regarding cirrhotic individuals survival and the importance of albumin. Individuals with cirrhosis are susceptible to the development of hydroelectrolytic disorders due to the pathophysiological alterations of the disease and because of the clinical status presented. Hyponatremia and hypokalemia are the most recurrent, but attention shall be given to the other disorders too. (AU)

ABSTRACT

RESUMEN El Sodio es uno de los principales cationes extracelulares que se encarga de controlar el volumen extracelular y la presión sanguínea. El Sodio ingresa al organismo de los alimentos y las bebidas y lo elimina principalmente en el sudor y en la orina. Una función renal intacta mantiene una concentración constante de Sodio, ajustando la cantidad eliminada en la orina; cuando la ingesta y la pérdida de Sodio no están en equilibrio, se altera la cantidad total de Sodio en el organismo. Estos cambios en las concentraciones de Sodio provocan trastornos del balance de agua. Tanto la hiponatremia (Na 145 mEq/L) causan principalmente síntomas neurológicos. La instauración de los síntomas relacionados con la hiponatremia está dada por la gravedad y rapidez del cambio de concentración plasmática del Sodio. En individuos sanos, la ingestión de agua no conduce a hiponatremia porque la liberación suprimida de hormona antidiurética (ADH) permite que el exceso de agua se excrete en una orina diluida. Esta revisión bibliográfica se basa en estudios y guías clínicas actualizadas, cuyo objetivo es facilitar de manera más resumida, práctica y esquemática la corrección de los trastornos del Sodio.


ABSTRACT Sodium is one of the main extracellular cations that is responsible for controlling extracellular volume and blood pressure. Sodium enters the body from food and drink and is eliminated mainly in sweat and urine. An intact kidney function maintains a constant sodium concentration, adjusting the amount excreted in the urine. When sodium intake and loss are not in balance, the total amount of sodium in the body is altered. These changes in sodium concentrations cause disturbances in the water balance. Both hyponatremia (Na 145 mEq/L) mainly cause neurological symptoms. The onset of symptoms related to hyponatremia is due to the severity and rapidity of the change in plasma sodium concentration. In healthy individuals, ingestion of water does not lead to hyponatremia because the suppressed release of antidiuretic hormone (ADH), allows excess water to be excreted in dilute urine. This bibliographic review is based on updated studies and clinical guidelines, the objective of which is to facilitate the correction of sodium disorders in a more summarized, practical and schematic way.

SUMMARY: The expression of aquaporin-1 (AQP1) in choroid plexus and aquaporin-4 (AQP4) in astrocyte of the hippocampal formation (HF) was studied in the rat to determine the role of AQP1 and AQP4 in the pathophysiology of systemic hyponatremia (SH). SH was induced by coadministration of dextrose solution intraperitoneally and through subcutaneous implantation of an osmotic minipump containing 8-deamino-arginin vasopressin (50ng/µl/h) for 24 and 48 h. Twenty- four and 48 h after the drug administration, there were significant reductions in Na+ concentration (111 ± 5 and 104 ± 2 mmol) and serum osmolarity (240 ± 13 and 221 ± 14 mOsm/L) as compared with control values (140 ± 4.7 mmol and 296 ± 5.2 mOsm/L), (p<0.01). The expression of AQP1 in the choroid plexus was increased three to five times from 24 h to 48 h after SH (329.86 ± 10.2 % and 531.5 ± 4.4 %, n=4, p<0.01). In contrast, AQP4 expression was significantly decreased up to 48 h after SH (36 ± 9 %, n=4, p<0.01). Quantitative immunoblotting revealed significant decreases of neuronal proteins in the HF after 24 to 48 h of SH. Therefore, we suggest that altered expression of AQP1 and AQP4 plays important role in the pathogenesis of systemic hyponatremia.


RESUMEN: En este análisis se estudió la expresión de acuaporina-1 (AQP1) en plexo coroideo y acuaporina-4 (AQP4) en astrocitos de la formación hipocampal (FH) en ratas para determinar el papel de AQP1 y AQP4 en la fisiopatología de la hiponatremia sistémica (HS). La HS fue inducida mediante la coadministración de solución de dextrosa por vía intraperitoneal y mediante la implantación subcutánea de una minibomba osmótica que contenía vasopresina 8-desaminoarginina (50 ng /µ l / h) durante 24 y 48 h. Veinticuatro y 48 h después de la administración del fármaco, hubo reducciones significativas en la concentración de Na + (111 ± 5 y 104 ± 2 mmol) y la osmolaridad sérica (240 ± 13 y 221 ± 14 mOsm /µL) en comparación con los valores de control (140 ± 4,7 mmol y 296 ± 5,2 mOsm / L), (p <0,01). La expresión de AQP1 en el plexo coroideo se incrementó de tres a cinco veces de 24 a 48 h después de HS (329,86 ± 10,2 % y 531,5 ± 4,4 %, n = 4, p <0,01). La inmunotransferencia cuantitativa reveló disminuciones significativas de proteínas neuronales en el FH después de 24 a 48 h de SH. Por lo tanto, sugerimos que la expresión alterada de AQP1 y AQP4 juega un papel importante en la patogénesis de la hiponatremia sistémica.

ABSTRACT

Convulsive seizures caused by hyponatremia occur when this condition is severe and develops quickly, resulting in a brain's adaptive inability to contain brain swelling. Seizures are rarely the cause of shoulder fractures. This is a case report of bilateral humerus fracture following a single epileptic seizure caused by drug hyponatremia, an unconventional event in medical practice. A 69-year-old woman was admitted to the emergency room after a single tonic-clonic seizure with spontaneously ceased sphincter relaxation, showing Glasgow 6. No falls or restraint were reported by observers. When alert, the patient reported pain and difficulty moving both arms. During examination, the movement was li- mited to the right and left. Anteroposterior radiographs revealed bilateral fracture at the neck of humerus. To complement inves- tigation for further lesions, a computed tomography confirmed bilateral fracture-dislocation with impaction of the humeral head with the glenoid. Atraumatic bilateral fracture-dislocation of the humerus after epileptic seizure is a very rare event. It is believed that some of these diagnoses have been neglected due to the difficulty of characterizing the patient's pain in a postictal state. The importance of a detailed physical examination shall be emphasized in risk groups such as the polymedicated elderly.

ABSTRACT

La hiponatremia es la alteración electrolítica más frecuente en el anciano y puede ser asintomática o producir un espectro de síntomas, especialmente del sistema nervioso central, tales como alteración del estado de conciencia, letargia, cefalea, convulsiones y alteraciones en la marcha, los cuales son un motivo frecuente de consulta de esta población. Esta entidad tiene un alto impacto en la funcionalidad del paciente, pues requiere múltiples hospitalizaciones, e incluso en mortalidad. Su etiología es multifactorial; entre sus causas más comunes están la baja ingesta de sal, las enfermedades crónicas como la nefropatía y la insuficiencia cardiaca y el síndrome de secreción inadecuada de hormona antidiurética (SIADH), que a su vez se produce comúnmente por el uso crónico de determinados fármacos, como los antidepresivos, los diuréticos y los antipsicóticos, que son los más olvidados en el abordaje clínico. Se presenta en este caso clínico el abordaje diagnóstico de la hiponatremia y la importancia de la anamnesis como instrumento clave para detectar la etiología de esta entidad clínica.


Hyponatraemia is the most common electrolyte disturbance in the elderly. It can be asymptomatic or produce a spectrum of symptoms, particularly in the central nervous system, such as altered state of consciousness, lethargy, headache, seizures and gait disturbances, all of which are a common reason for consultation in this population. This condition has a high impact on the functionality of the patient given the need formultiple hospital stays, as well as on mortality. Its aetiology ismultifactorial and its most common causes include low salt intake, chronic diseases such as kidney disease and heart failure, and the syndrome of inappropriate antidiuretic hormone secretion (SIADH), which is commonly caused by the chronic use of certain drugs, such as antidepressants, diuretics and antipsychotics, which are the most forgotten in clinical practice. The following clinical case presents the diagnostic approach of hyponatraemia and the importance of the medical history as a key tool to detect the aetiology of this clinical entity.

ABSTRACT

Abstract The combination of trimethoprim-sulfamethoxazole (TMP-SMZ or cotrimoxazole) has a bactericidal effect on gram-positive cocci and gram-negative bacilli. It is used clinically for skin and soft tissue, respiratory and urinary tract infections, and is also relevant for prophylaxis and treatment of opportunistic infections in immunosuppressed patients. Its use at established doses in immunocompetent patients is safe, with a low rate of adverse events. However, in immunosuppressed individuals (human immunodeficiency virus [HIV], transplants, or steroid users), the adverse effects (AEs) of this medication reach 83%; and, when administered parenterally at high doses, lactic acidosis has been documented secondary to the polyethylene glycol vehicle. We present two cases of patients who ingested an overdose of TMP-SMZ and developed type 2 renal tubular acidosis (RTA), which has not been described with this medication, and whose hyperlactatemia is not explained by the polyethylene glycol excipient, as it was taken orally.


Resumen La combinación de trimetoprim-sulfametoxazol (TMP-SMZ o cotrimoxazol) tiene efecto bactericida sobre cocos gram positivos y bacilos gram negativos, con uso clínico en infecciones de piel y tejidos blandos, del tracto respiratorio y urinario, además con relevancia en la profilaxis y tratamiento de infecciones oportunistas en pacientes inmunosupresos. Su uso en pacientes inmunocompetentes a dosis establecidas es seguro, con una baja tasa de eventos adversos. Sin embargo, en población con inmunosupresión: virus de inmunodeficiencia humana (VIH), trasplante, o usuarios de esteroides; los efectos adversos (EA) por este medicamento alcanzan 83% y por administración parenteral en dosis elevadas se ha documentado acidosis láctica secundaria al vehículo polietilenglicol. Presentamos dos casos de pacientes que ingirieron TMP-SMZ en sobredosis, desarrollando acidosis tubular renal (ATR) tipo 2, la cual no se ha descrito en este medicamento y cuya hiperlactatemia no es explicable por el excipiente polietilenglicol debido a que el consumo fue oral.

ABSTRACT

RESUMEN La administración de líquidos y electrolitos parenterales es una terapia fundamental de soporte en niños agudamente enfermos si la vía oral no permite la administración de la cantidad o composición requeridas de fluidos, por ejemplo en enfermedades de tipo gastrointestinal, respiratorio, neurológico, o en el período transoperatorio. Al momento de planificar la administración de líquidos y electrolitos parenterales en niños a partir del mes de edad, se debe considerar componentes relacionados con la cantidad de líquidos a infundir considerando los requerimientos secundarios a pérdidas hídricas habituales o requerimientos para reacciones metabólicas, evaluar el grado de deshidratación para la administración de líquidos que complementen el déficit, corregir las pérdidas que se han producido secundarios a una noxa externa (enfermedad, procedimiento quirúrgico, trauma, etc.) y finalmente aportar la cantidad adecuada de fluido que permita re-establecer la perfusión tisular. Es necesario conocer los cambios instaurados basados en la evidencia actual con la finalidad de incorporar a el manejo frecuente de los pacientes considerando también la toxicidad ya sean cualitativos o cuantitativos de esta terapéutica tan necesaria en el manejo del paciente pediátrico


ABSTRACT The administration of parenteral fluids and electrolytes is a fundamental support therapy in acutely ill children if the oral route does not allow the ad-ministration of the required amount or composition of fluids, for example in gastrointestinal, respiratory, neurological, or transoperative diseas-es. When planning the administration of parenteral fluids and electrolytes in children from one month of age onwards, components related to the amount of fluids to be infused should be considered, taking into account the requirements secondary to habitual water loss or requirements for meta-bolic reactions, evaluating the degree of dehydration for the administration of fluids to supplement the deficit, correcting the losses that have oc-curred secondary to an external noxa (illness, surgical procedure, trauma, etc.) and finally providing the adequate amount of fluid to allow re-es-tablishment of tissue perfusion. It is necessary to know the changes established based on current evidence in order to incorporate to the frequent management of patients, also considering the toxicity, either qualitative or quantitative, of this therapy, so necessary in the management of the pediatric patient.

Resumen La hiponatremia es el trastorno hidroelectrolítico más frecuente observado en pacientes hospitalizados y es importante resaltar que se ha asociado a morbilidad y mortalidad de estos. Esta entidad representa un proceso fisiopatológico relacionado con una alteración en la homeostasis del agua, en la cual los pacientes presentan síntomas en su mayoría neurológicos, que se correlacionan con el nivel de sodio y el tiempo de aparición del trastorno. Se realizó una búsqueda en las bases de datos PubMed y Lilacs de monografías, artículos de revisión y artículos originales con el objetivo de revisar aspectos sobre la clínica, diagnóstico y manejo de la entidad. Para el diagnóstico, la batería de estudios se solicita en función del contexto clínico, actuando temprano y permitiendo la corrección del trastorno de acuerdo al escenario respectivo. La hiponatremia es un problema médico frecuente que bajo un abordaje práctico y sencillo permite tomar decisiones clínicas de forma oportuna. MÉD.UIS. 2020;33(2):85-93.


Abstract Hyponatremia is the most frequent hydroelectrolytic disorder observed in hospitalized patients and it is important to note that it has been associated with morbidity and mortality in this patients. This entity represents a pathophysiological process related to an alteration in water homeostasis, in which patients present symptoms mostly neurological, that correlate with the sodium level and the time of onset of the disorder. A search of the PubMed and Lilacs databases of monographies, review articles and originals articles was performed with the objective to review clinical, diagnostic and management aspects of this entity. For diagnosis, the clinical laboratory studies are requested depending on the clinical context, acting early and allowing correction of the disorder according to the respective scenario. Hyponatremia is a frequent medical problem that requires a practical and simple approach favoring clinical decisions in a timely manner. MÉD.UIS. 2020;33(2):85-93.
